I second Juliette - although Sevilla, Granada, Cordoba, etc would are lovely, it's seriously hot down there. Even the flies lay down and quietly gasp for shade and water. If that's your thing, though, don't let it put you off, esp as Granada's Alhambra is a World Heritage Site.
Toledo is also lovely, and close to Madrid. It too will be hot, but that's virtually all of Espana. Salamanca or Caceres would give you a feel for an old-world Spanish village. If you go to either city, make sure to see it at night, esp Plaza Mayor in Salamanca.
I also encourage San Sebastian, one of the prettiest cities in Spain, as well as one of the nicest-feeling. The mountains and coasts are lovely too, with more Spanish tourists and less "guiris".
And Barcelona...well, it's not like if you come to Madrid then there's not much reason to see Barcelona. The cities are so different in character, landscape, people, even food and language. And there's always side trips from there, too. I always like to go out to Girona when I'm there, a pretty medieval town about 1.5 hrs by train.
I've also been recommended Cadiz in the south, and all the Spanish swear by Galicia in the northwest. Guess it depends on how much time and money you have.
If it were me, and I were visiting (knowing what I know how) I'd take a trip out to Toledo and Segovia, go on to Barcelona. If it weren't summer, I'd try to get Sevilla and/or Granada in, too, since the train ride down there on the AVE isn't very long, like two or three hours max.
