I don't know of any English-speaking midwives, but there are a couple of English-speaking doulas in Madrid.

To avoid the horror stories, avoid public health services and go private. Hopefully you had private insurance prior to becoming pregnant, such as Sanitas or Adeslas.
The best bet is to contract a natural birthing center to ensure caring and respectful treatment. I (and board master jer) used
www.nacentia.com with Dr. Regina Cardenas. Apart from promoting natural birth, they promote an environment of respect for the mother and caring for the entire family. For example, I ended up having a cesarean and the two doctors who did it, I knew by first name. They talked to me throughout the procedure. Meanwhile the midwife Diego, kept me informed about all the details of the baby and also ran out to my husband with detials. He also dressed and brought the baby to my husband while I was getting sewn up. At the same time, my doula, Mercedes held my hand throughout the procedure and then helped me hold the baby directly after.
Public hospitals will do none of this!
